Schistosomiasis

Definisjon:
Er en innvollsorm. Smitten skjer via kontaminert vann, larveformen kan smitte mennesker gjennom hud eller slimhinner
Forekomst:
Er vanlig i tropiske strøk, og er et betydelig globalt problem. Ses sporadisk i Norge, fortrinnsvis hos innvandrere
Symptomer:
Det kan bli urtikarielt utslett der larvene penetrerer huden. 2-6 uker etter eksponering kan symptomer som magesmerter, vekttap, hodepine, sykdomsfølelse, tørrhoste, frysninger, feber, myalgi og (ev. blodig-) diaré oppstå. I kronisk fase kan sykdommen kan gi symptomer fra blære (bl.a. hematuri), CNS, lever osv.
Funn:
Hepatomegali kan oppstå allerede i akutt fase, og etterhvert kan også milten palperes som forstørret
Diagnostikk:
Eggene påvises i mikroskopisk undersøkelse av avføring og urin, PCR kan bekrefte dette. Ev. anemi og eosinofili i akuttfasen. Ulike bildeundersøkelser kan avsløre komplikasjoner av infeksjonen
Behandling:
Medikamentell behandling er effektivt for de fleste. Kirurgisk behandling kan være nødvendig i tillegg.
