Divertikkelsykdom

Definisjon:
En tilstand hvor kolonmucosa hernierer gjennom muscularis. Skyldes endringer i kolonveggens motstandsevne, forstyrret kolonmotilitet, lavt fiberinntak og genetiske forhold
Forekomst:
De fleste er asymptomatiske. Prevalens øker med alderen og var i en studie 30% blant 50-åringer og 70% blant 80-åringer
Symptomer:
Bare 10-20% av dem med divertikulose har symptomer. Av disse utvikler 3/4 divertikulitt og av disse igjen får 1/3 komplikasjoner. Smertefri rektal blødning forekommer, noen ganger alvorlig forløp. Andre symptomer er avføringsendringer, venstresidige kolikksmerter som lindres ved defekasjon og luftplager
Funn:
Vanligvis ingen kliniske funn ved divertikkelsykdom. Ved divertikulitt foreligger lokal ømhet i nedre v. fossa, stramming, direkte palpasjonsømhet og feber
Diagnostikk:
CT kolon/bekken viktigst
Behandling:
Divertikkelsykdom krever vanligvis ikke annen behandling enn informasjon. Ved ukomplisert divertikulitt anbefales ikke antibiotika. Innleggelse i sykehus ved komplisert divertikulitt
