Akutt infeksiøs encefalitt (AIE)

Definisjon:
Akutt inflammasjon i hjernen med assosierte nevrologiske symptomer. Årsaken er ofte infeksiøs, og viral etiologi er vanlig. De fleste ikke-infeksiøse encefalitter er immunologisk betinget
Forekomst:
Ca. 2/100.000 og tilstanden ses hyppigst hos barn og unge voksne. Alvorlighetsgraden er størst blant spedbarn og eldre
Symptomer:
Både akutt innsettende symptomer og gradvis utvikling over dager er vanlig. Encefalopati av en eller annen art er vanligste symptom
Funn:
Generelle infeksjonstegn med hjerneparenkym-symptomer og evt. meningeal irritasjon
Diagnostikk:
Mikrobiologisk diagnose kan være vanskelig å stille, men nevrologiske manifestasjoner og patologisk CSF bekrefter diagnosen
Behandling:
Tidlig empirisk terapi med intravenøs aciclovir er påkrevet ved alle mistenkte encefalitter. Mange pasienter med gjennomgått encefalitt har betydelige sekveler
