Epilepsi hos barn

Definisjon:
Epilepsi er en samlebetegnelse på en rekke tilstander med tilbakevendende epileptiske anfall som fellesnevner. Anfallene skyldes cerebrale funksjonsforstyrrelser og inndeles i fokale anfall, generaliserte anfall og anfall med ukjent start.
Forekomst:
Insidensen av epilepsi er høyest i første leveår og ≥ 65 år. Prevalensen i alle aldersgrupper ca. 0,7% i Norge, som inkluderer rundt 5200 barn.
Symptomer:
Epileptiske anfall. Man skiller mellom forskjellige typer generaliserte anfall og fokale anfall med bevart eller redusert bevissthet. Utformingen avhenger av hvor i hjernen anfallene starter. Fokale anfall kan av og til utvikle seg til store krampeanfall (tonisk-kloniske anfall). Slike kan også opptre alene, uten fokal start (generaliserte tonisk-kloniske anfall). Det finnes dessuten mindre anfall uten kramper, for eksempel kan barn ha korte fjernhetsanfall (absenser, en type generaliserte anfall).
Funn:
Klinisk undersøkelse utenom anfall er som oftest normal, men kan også avdekke nevrologiske utfall.
Diagnostikk:
Diagnosen er først og fremst basert på grundig sykehistorie, inkl. komparentopplysninger, klinisk undersøkelse, orienterende blodprøver, EEG og cerebral MR. Ved behov kan infeksjonsmedisinsk, immunologisk, genetisk, metabolsk, hematologisk, kardiologisk eller nevropsykologisk utredning være indisert i tillegg.
Behandling:
Behandlingen består av informasjon, veiledning, medikamentell behandling, forebygging av bivirkninger, rehabilitering, ev. kirurgi, nervestimulering, ketogen diett. Viktig med fornyet diagnostisk vurdering tidlig i sykdomsforløpet hos farmakoresistente tilfeller.
