Panikklidelse

Definisjon:
Flere, uventede, plutselige og ofte uforklarlige panikkanfall med bl.a. hjertebank, skjelving, lufthunger, svimmelhet og brystsmerter som typisk fører til angst for nye anfall og livsforstyrrende unngåelsesatferd. Ikke sjelden opplever pasienten dødsangst
Forekomst:
Livstidsprevalensen for panikklidelse er 1-5%, mens nesten 1 av 3 vil oppleve ett panikkanfall. Tilstanden forekommer dobbelt så hyppig blant kvinner
Symptomer:
Kortvarige anfall (5-30 min) der vedkommende plutselig og tilsynelatende uten grunn blir redd med symptomer på adrenerg stimulering
Funn:
Normale kliniske funn utenfor anfall. Funn som kan tilskrives somatisk adrenerg stimulering under anfall
Diagnostikk:
Ingen tilleggsundersøkelser er av diagnostisk verdi
Behandling:
Kognitiv terapi med eksponering er den best dokumenterte behandlingen. Psykodynamisk terapi, psykologisk støttebehandling og forklaring av anfallsopplevelser samt antidepressiva kan være nyttig. Terapi kan løse panikkproblemene hos 80-90%
